Benefit Plan Changes

Medical Reimbursement Plan members only. All non-emergency services obtained from a non-PPO provider will be reduced 50%. Outpatient surgery performed at a non-PPO facility is limited to $2,500.00

Eligibility Change

 
This change applies only to Active participants. Coverage for all terminated participants will cease at the end of the month following the last month worked and paid for by your employer.

Applies to all Actives and Retirees. New Department of Labor regulations on Urgent Care authorizations and shortened time limitations for the Plan to review your appeal.

Women's Health & Cancer Rights Act Of 1998

Annual Notice – Applies to all Actives and Retirees: The Women’s Health and Cancer Rights Act of 1998 (WHRCA) is a federal law that provides protection to patients who choose to have breast reconstruction in connection with a mastectomy.

Domestic Partner Coverage - Same Sex

Applies to all Actives and Retirees.  Qualified Domestic Partner health coverage allows participants to enroll their same-sex domestic partners for health plan coverage with the Teamsters Miscellaneous Security Trust Fund (TMSTF) effective January 1, 2005.   The rules governing coverage of same-sex domestic partners are outlined in this notice.

Merger of Plan G-N & Plan G-O

Notice to all Plan G-N participants advising the merger of Plan G-N to Plan G-O effective October 1, 2005.

Out-of-Pocket Maximum Notice

Applies to all active Plan G-O participants eligible under the Medical Reimbursement Plan. Notice advising the new out-of-pocket maximum of $1,500 effective with October 1, 2005 service dates.

Employee Member Assistance Program (EMAP)

Applies to all Active and Retired participants enrolled in PacifiCare, Health Net or the Medical Reimbursement Plan. Effective March 31, 2007 Health Management Concepts and APS Healthcare (HMC/APS) will replace will replace Managed Health Network as the new Mental Health and Chemical Dependency service provider. All members enrolled in Kaiser will continue to receive their Mental Health and Chemical Dependency benefits from Kaiser..
